摘要:
目的对比静脉溶栓与腔内治疗急性肺栓塞的近期效果。方法回顾性分析2018年7月~2020年9月我院68例急性肺栓塞资料。在抗凝治疗的基础上,联合静脉溶栓38例,采用低分子肝素抗凝、静脉阿替普酶溶栓;联合腔内治疗30例,采用抽吸取栓、导管碎栓、局部溶栓的综合治疗。比较2组治疗效果、死亡率及并发症。结果腔内治疗组有效率高[100%(30/30) vs. 86.8%(33/38),P=0.062],死亡率和出血并发症发生率低[0(0/30) vs. 7.9%(3/38),P=0.249;67%(2/30) vs. 18.4%(7/38), χ2=1.123,P=0.289],但差异无统计学意义(P>0.05)。腔内治疗组住院时间短于静脉溶栓组[(8.9±2.0) d vs. (12.3±3.5) d,t=-4.839,P=0.000]。治疗后2组心率、肺动脉压力均明显下降,氧饱和度明显上升(P<0.01),腔内治疗组更明显(P<0.01)。结论静脉溶栓和腔内治疗均可有效治疗急性肺栓塞,腔内治疗更为有效和安全。
Abstract:
ObjectiveTo compare the effectiveness and safety of endovascular treatment and intravenous thrombolysis of acute pulmonary embolism.MethodsThe data of 68 cases of acute pulmonary embolism in our hospital from July 2018 to September 2020 were retrospectively analyzed. On the basis of anticoagulant therapy, 38 cases were given anticoagulation treatment with low molecular weight heparin and intravenous thrombolysis with alteplase, and 30 cases were treated with combined endovascular therapy, including thrombus aspiration, catheter broken and local thrombolysis. The effect, mortality and complications of the two groups were compared.ResultsThe mortality and the incidence of bleeding complication in endovascular group were lower than those in intravenous thrombolysis group [0(0/30) vs. 7.9%(3/38), P=0.249; 6.7%(2/30) vs. 18.4%(7/38), χ2=1.123, P=0289], without statistically significant difference (P>0.05). The length of hospital stay in endovascular group was shorter than that in intravenous thrombolysis group [(8.9±2.0) d vs. (12.3±3.5) d, t=-4.839, P=0.000]. After treatment, the heart rate and pulmonary artery pressure decreased and oxygen saturation increased significantly in both groups (P<0.01), and more obvious effect observed in the endovascular group (P<0.01).ConclusionBoth intravenous thrombolysis and endovascular therapy can effectively treat acute pulmonary embolism, but endovascular treatment is more effective and safe.
