目的初步探讨纳米炭示踪剂在机器人辅助胃癌根治术中的应用价值。方法回顾性分析2019年11月~2020年7月78例达芬奇机器人辅助胃癌D2根治术资料,其中纳米炭组41例,对照组37例。纳米炭组术前1天胃镜下距肿瘤边缘0.5~1 cm处分四点(口侧、肛侧、大弯侧、小弯侧)注射纳米炭混悬液共1 ml(50 mg),2组均采用达芬奇Xi机器人行胃癌D2根治术。比较2组淋巴结检出情况和手术指标。结果纳米炭组检出淋巴结总数多[(40.8±5.2)枚vs. (35.4±7.2)枚,t=3.868,P=0.000],检出微小淋巴结(直径<2 mm)多[(8.8±4.8)枚vs. (4.1±1.7)枚,t=5.960,P=0.000],2组非微小淋巴结的检出数量及淋巴结转移率差异无统计学意义(P>0.05)。纳米炭组手术时间短[(223.9±22.8)min vs. (255.4±31.7)min,t=-5.000,P=0.000]。结论纳米炭可以增加机器人辅助胃癌根治术的淋巴结检出总数,尤其提升微小淋巴结检出数目,从而提高胃癌淋巴结分期的准确性。
Abstract:
ObjectiveTo discuss the application value of carbon nanoparticle suspension injection (CNSI) in robotassisted radical gastrectomy for gastric cancer.MethodsClinical data of 78 patients of robotassisted D2 radical gastrectomy for gastric cancer between November 2019 and July 2020 were respectively reviewed. Among these patients, there were 41 in the CNSI group and 37 in the control group. In the CNSI group, 1 ml (50 mg) of carbon nanoparticle suspension was injected submucosally at four points (oral side, anal side, large curvature side and small curvature side) 0.5-1 cm away from the tumor edge under endoscopy one day before operation. D2 radical gastrectomy was performed by using the da Vinci Xi Surgical System in both groups. The lymph node and operative index were compared.ResultsThe total number of lymph nodes in the CNSI group was more than the control group [(408±5.2) nodes vs. (35.4±7.2) nodes, t=3.868, P=0.000], and the mean number of micro lymph nodes (<2 mm in diameter) was more than the control group [(8.8±4.8) nodes vs. (4.1±1.7) nodes, t=5.960, P=0.000]. There was no significant difference in the number of nonmicro lymph nodes and lymph node metastasis rate between the two groups (P>0.05). The operation time of the CNSI group was shorter than the control group [(223.9±22.8) min vs. (255.4±31.7) min, t=-5.000,P=0.000].ConclusionThe application of CNSI in robotassisted radical gastrectomy for gastric cancer can increase the total number of lymph nodes retrieved, especially the number of micro lymph nodes retrieved, to further improve the accuracy of lymph node staging for gastric cancer.
