摘要:
目的系统评价奥赛利定对比舒芬太尼在术后患者自控静脉镇痛(patientcontrolled intravenous analgesia，PCIA)中的呼吸抑制风险。方法检索万方数据、维普中文期刊数据库、中国知网(CNKI)、中国生物医学文献数据库(CBM)、PubMed、Web of Science、Embase数据库，搜集关于奥赛利定对比舒芬太尼用于PCIA的随机对照试验(RCT)，检索时限均为建库至2026年5月22日。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险，采用RevMan 5.4软件进行Meta分析。结果共纳入8项RCT，包括3篇英文文献和5篇中文文献，共850例。Meta分析结果显示：①主要结局指标：术后48 h内呼吸抑制风险奥赛利定组显著低于舒芬太尼组(RR=0.34，95% CI：0.18~0.64，P<0.001)。②次要结局指标：术后48 h内恶心呕吐风险奥赛利定组低于舒芬太尼组(RR=0.47，95% CI：0.36~0.62，P<0.001)；术后12 h和48 h活动状态疼痛视觉模拟评分(Visual Analog Scale，VAS)奥赛利定组低于舒芬太尼组(MD=-0.24，95% CI：-0.31~-0.16，P<0.001；MD=-0.24，95% CI：-0.46~-0.02，P=0.03)，术后12、24、48 h静息状态及术后24活动状态VAS评分2组差异均无统计学意义(P>0.05)；术后补救镇痛次数2组差异无统计学意义(P>0.05)；术后24 h及48 h恢复质量评分(15item Quality of Recovery，QoR15)奥赛利定组均高于舒芬太尼组(P<0.001)。结论在PCIA中，奥赛利定呼吸抑制风险显著低于舒芬太尼，同时可降低恶心呕吐风险，且镇痛效果与舒芬太尼相当。
Abstract:
ObjectiveTo systematically review the risk of respiratory depression between oliceridine and sufentanil in postoperative patientcontrolled intravenous analgesia (PCIA).MethodsDatabases including Wanfang Data, VIP, CNKI, CBM, PubMed, Web of Science and Embase were electronically searched from inception to May 22, 2026 to collect randomized controlled trials (RCT) comparing oliceridine versus sufentanil for postoperative analgesia. Two researchers independently screened literature, extracted data, and assessed the risk of bias of included studies. Metaanalysis was performed using the RevMan 5.4 software.ResultsA total of 8 randomized controlled trials involving 3 English articles and 5 Chinese articles with a total of 850 patients were included. The metaanalysis results showed as follows. ①Primary outcome: The risk of respiratory depression within 48 h after surgery was significantly lower in the oliceridine group than in the sufentanil group (RR=0.34, 95%CI: 0.18~0.64, P<0.001). ②Secondary outcomes: The risk of postoperative nausea and vomiting within 48 h after surgery was lower in the oliceridine group than in the sufentanil group (RR=0.47, 95%CI: 0.36~0.62, P<0.001). The Visual Analog Scale (VAS) scores for dynamic pain at 12 h and 48 h postoperatively were significantly reduced in the oliceridine group (MD=-0.24, 95%CI: -0.31~-0.16, P<0001; MD=-0.24, 95%CI: -0.46~-0.02, P=0.03). No statistically significant intergroup differences were observed in resting VAS scores at 12 h, 24 h, and 48 h postoperatively and dynamic VAS scores at 24 h postoperatively (P>0.05). There was no significant difference in the number of rescue analgesic administrations between the two groups (P>0.05). The 15item Quality of Recovery (QoR15) scores at 24 h and 48 h after surgery were markedly higher in the oliceridine group than in the sufentanil group (P<0.001).ConclusionDuring postoperative PCIA, oliceridine is associated with a significantly lower risk of respiratory depression and a reduced risk of nausea and vomiting compared with sufentanil, while both agents yield comparable analgesic efficacy.
