摘要:
目的比较关节镜下2种固定方式(带袢钛板与高强度缝线)治疗急性成人后交叉韧带(posterior cruciate ligament，PCL)胫骨止点撕脱骨折的早期疗效。方法回顾性分析2022年1月~2024年3月41例双后内入路关节镜手术治疗急性PCL胫骨止点撕脱骨折并随访6个月资料，其中20例采用带袢钛板固定(钛板组)，21例采用高强度缝线固定(缝线组)。比较2组手术时间、切口愈合、并发症，以及术后6个月Lysholm膝关节评分和国际膝关节文献委员会(International Knee Documentation Committee，IKDC)评分。结果钛板组手术时间［(51.9±9.3)min］短于缝线组［(64.2±11.5)min］(t=-3.770，P=0.001)。钛板组和缝线组术后6个月Lysholm评分分别为(94.3±3.4)分和(93.1±3.5)分，均较术前的(48.2±5.0)分和(47.6±4.4)分显著提高(均P=0.000)；钛板组和缝线组术后6个月IKDC总分分别为(95.1±2.8)分和(93.8±2.9)分，均较术前的(48.0±3.4)分和(46.3±4.7)分显著提高(均P=0.000)。2组术后6个月膝关节Lysholm、IKDC评分差异无显著性(P>0.05)。结论双后内入路关节镜治疗急性PCL胫骨止点撕脱骨折术中，带袢钛板与高强度缝线固定均有创伤小、恢复快、早期效果满意等优点，高强度缝线固定的手术时间较长。
Abstract:
ObjectiveTo compare the early therapeutic effects of two fixation methods (loop titanium plate and highstrength suture) for acute adult posterior cruciate ligament (PCL) avulsion fractures at the tibial insertion under arthroscopy.MethodsA retrospective analysis was conducted on the data of 41 cases of acute PCL tibial insertion avulsion fractures treated by the double posterior internal approach arthroscopic surgery from January 2022 to March 2024, with a followup period of 6 months. Among them, 20 cases were fixed with a looped titanium plate (titanium plate group), and 21 cases were fixed with highstrength sutures (suture group). The surgical time, incision healing, complications, as well as the Lysholm knee joint score and the International Knee Documentation Committee (IKDC) score at 6 months after surgery were compared between the two groups.ResultsThe operation time of the titanium plate group ［(51.9±9.3) min］ was shorter than that of the suture group ［(64.2±11.5) min; t=-3.770, P=0.001］. The Lysholm scores of the titanium plate group and the suture group at 6 months after surgery were (94.3±3.4) points and (93.1±3.5) points respectively, both significantly higher than the preoperative scores of (48.2±5.0) points and (47.6±4.4) points (both P=0.000). The IKDC total scores of the titanium plate group and the suture group at 6 months after surgery were (951±2.8) points and (93.8±2.9) points respectively, both significantly higher than the preoperative scores of (48.0±3.4) points and (46.3±4.7) points (both P=0.000). There was no significant difference in the Lysholm and IKDC scores of the knee joint between the two groups at 6 months after surgery (P>0.05).ConclusionsIn the arthroscopic treatment of acute PCL tibial insertion avulsion fracture using the double posterior internal approach, both the looped titanium plate fixation and the highstrength suture fixation have advantages of less trauma, faster recovery, and satisfactory early results. However, the surgery time for the highstrength suture fixation is longer.
