摘要:
目的观察成人正常颈椎关节突关节的三维解剖形态,利用影像归档和通信系统(Picture Archiving and Communication Systems,PACS)测量其三维参数并探讨其临床意义,为后期颈椎小关节界面撑开器的设计与制作提供可靠的依据。方法选取2019年9月~2020年5月在我院影像科行颈椎三维螺旋CT检查200例,螺旋CT图像显示无颈椎椎管狭窄、颈椎椎间盘突出、明显骨质增生、感染及肿瘤等病变,经三维螺旋CT重建后分别在矢状面、横断面及冠状面对C2~3、C3~4、C4~5、C5~6、C6~7 5个节段两侧关节突关节前后径、关节间隙间距及左右径进行测量。结果自上而下颈椎小关节男性前后径为8.5~13.0 mm,关节间隙间距为0.7~1.5 mm,左右径为9.0~135 mm;女性前后径为8.0~12.0 mm,关节间隙间距0.5~1.2 mm, 左右径为8.0~12.5 mm。各节段关节突关节三维参数男性均明显大于女性(P<0.05),且随着颈椎节段向下依次增大。结论颈椎关节突关节的前后径、关节间隙距、左右径长度在各个节段以及不同性别间各不相同。
Abstract:
ObjectiveTo study the threedimensional structure of the cervical zygapophyseal joint in adults, measure the joint anatomic parameters by the Picture Archiving and Communication Systems (PACS), and discuss its clinical significance, to provide reliable data for the design of the cervical facet retraction device.MethodsThe study included 200 cases that were admitted in our hospital from September 2019 to May 2020. All the patients had received cervical CT threedimensional reconstruction scanning, none of whom showed spinal stenosis, cervical disc herniation, hyperostosis, infections or tumors. After threedimensional CT reconstruction, the anterior and posterior diameter, the distance between the joint space and the left and right diameter of the zygapophyseal joint of C2-3, C3-4, C4-5, C5-6 and C6-7 segments were measured in sagittal, coronal and horizontal planes, respectively.ResultsThe anterior and posterior diameter of the male facet joint from top to lower cervical vertebrae was 8.5-13.0 mm, the distance between the joint space was 0.7-1.5 mm, the distance between the left and right joint was 9.0-13.5 mm; the anterior and posterior diameter of the female facet joint was 8.0-12.0 mm, the distance between the joint space was 0.5-1.2 mm, and the left and right diameter was 8.0-12.5 mm. The threedimensional parameters of facet joints in males were larger than those in females (P<0.05), and with the cervical segment down in turn increased.Conclusion The anterior and posterior diameter, the distance between the joint space and the left and right diameter of the zygapophyseal joint are different in each segment and between males and females.
