目的探讨应用专利器械腔镜接骨板行完全电视胸腔镜肋骨骨折记忆合金内固定的可行性和安全性。方法2016年10月~2019年6月,使用专利器械腔镜用经胸腔记忆合金肋骨接骨板及胸腔镜肋骨接骨板植入工具,采取施夹钳法或穿刺牵引悬吊法,完成31例肋骨骨折完全胸腔镜内固定术。创伤性浮动胸壁9例;单纯多发性肋骨骨折断端有明显移位22例,其中9例骨折断端位于肩胛区或脊柱旁区。结果均在完全胸腔镜下完成手术,无并发症发生。术后3个月均达到临床愈合标准。随访6~24个月,平均12个月,无明显胸部不适症状,未发生近期及远期内固定装置脱落,两侧胸廓基本对称,无胸廓塌陷畸形。结论完全胸腔镜肋骨骨折记忆合金内固定术的应用,使胸腔镜处理胸部创伤的同时可以进行肋骨骨折内固定,是一种安全、有效、微创的方法。
Abstract:
ObjectiveTo investigate the feasibility and safety of total thoracoscopic memory alloy internal fixation for rib fractures with patented thoracoscopic rib plate. MethodsFrom October 2016 to June 2019, a total of 31 patients with traumatic rib fractures were treated with total videoassisted thoracoscopic internal fixation by using thoracoscopic rib plate of memory alloy and thoracoscopic rib plate implantation tool. Internal fixation of rib fractures was performed by means of thoracoscopic clamp or puncture traction suspension. There were 9 cases of traumatic flail chest wall, as well as 22 cases of multiple rib fractures with obvious displacement of the fracture end, among which 9 cases were located in the scapular area or the paraspinal area.ResultsTotal thoracoscopic surgery was performed in all the patients without complications. The clinical healing standard was reached 3 months after operation. All the patients were followed up for 6-24 months, with an average of 12 months. There was no obvious chest discomfort. No recent or longterm internal fixation device fallingoff happened. Both sides of the thorax were basically symmetrical, and there was no chest collapse deformity.ConclusionsAcute thoracic trauma can be treated with videoassisted thoracoscopic internal fixation of rib fractures at the same time. Total videoassisted thoracoscopic memory alloy internal fixation for rib fracture is a safe, effective and minimally invasive method.
