摘要:
目的总结处女膜中央椭圆形切开联合切缘连续锁边缝合治疗青春期处女膜闭锁(imperforate hymen，IH)的初步经验。方法2019年9月~2024年10月，对16例IH(12岁~13岁2个月，中位年龄12岁5个月)实施处女膜切开术。处女膜中央椭圆形切口，吸除阴道和子宫腔内的积血，40 PDS缝线从椭圆形切口12点方向逆时针全层连续锁边缝合，观察2 min，未见切缘活动性出血或渗血。结果本组手术时间20~25 min(平均22 min)，术中吸出阴道和子宫腔内积血300~800 ml，无术中并发症。术后4~6 h下地活动，术后24 h出院。在门诊和线上随访12个月~2年3个月(平均18个月)，月经正常，无处女膜再次粘连和闭合，家长对手术效果均满意。结论处女膜中央椭圆形切开联合切缘连续锁边缝合治疗青春期IH，在操作上具有安全性、有效性，是对处女膜闭锁外科手术方法的补充。
Abstract:
ObjectiveTo present our initial experience of central elliptical incision of the hymen combined with continuous locked suture of the incision margin for the treatment of adolescent imperforate hymen (IH).MethodsFrom September 2019 to October 2024, 16 patients with IH (age range, 12 years-13 years 2 months old; median, 12 years 5 months old) underwent hymenoplasty. A central elliptical incision was made on the hymen. The accumulated blood within the vagina and uterine cavity was slowly aspirated. The hymenal incision edges were closed using a 40 PDS suture with a continuous locked suture technique, starting at the 12 o’clock position and proceeding counterclockwise. After observing for 2 min, no active bleeding or oozing from the wound edges was observed.ResultsThe operative time was 20-25 min (mean, 22 min). The volume of aspirated blood within the vagina and uterine cavity was 300-800 ml. No intraoperative complications occurred. All the patients were ambulatory 4-6 h postoperatively and were discharged 24 h after surgery. During outpatient and online followups for 12 months to 2 years and 3 months (mean, 18 months), menstruation was normal and there was no readhesion or closure of the hymen. Their parents were satisfied with the surgical outcomes.ConclusionsCentral elliptical hymenal incision combined with continuous locked suture of the incision margin is a safe and effective procedure for the treatment of adolescent IH. It represents a valuable supplement to existing surgical methods for imperforate hymen.
