目的探讨ATEC活检系统在可疑乳腺癌钙化灶X线立体定位真空辅助活检(stereotactic vacuumassisted biopsy,SVAB)中的应用价值。方法2018年7月~2019年12月198例(215侧乳房)因乳腺钙化灶使用ATEC活检系统行SVAB,回顾性评估该活检系统的有效性、安全性及组织学低估率。结果215侧乳房均顺利完成活检,病理示良性病变165侧,非典型导管上皮增生19侧,恶性病变31侧(其中导管原位癌25侧,导管原位癌伴局灶浸润性导管癌3侧,浸润性导管癌2侧,浸润性微乳头状癌1侧)。组织学低估率导管原位癌为16.0%(4/25),非典型导管上皮增生为5.3%(1/19)。结论ATEC系统可以较好地应用于中国女性常见可疑钙化灶微创活检,值得推广。
Abstract:
ObjectiveTo explore the application value of ATEC biopsy system in stereotactic vacuumassisted biopsy (SVAB) of suspicious breast cancer calcification. MethodsFrom July 2018 to December 2019, 198 cases (215 breasts) underwent SVAB using the ATEC biopsy system for breast calcification. The effectiveness, safety and underestimation rate of the biopsy system were retrospectively evaluated.ResultsAll the 215 breasts were successfully biopsied. Surgical pathology showed 165 breasts of benign lesions, 19 breasts of atypical ductal hyperplasia, and 31 breasts of malignant lesions (including 25 breasts of ductal carcinoma in situ, 3 breasts of ductal carcinoma in situ with focal invasive ductal carcinoma, 2 breasts of invasive ductal carcinoma, 1 breast of invasive micropapillary carcinoma). The histological underestimation rate of ductal carcinoma in situ was 16.0% (4/25), and the atypical ductal epithelial hyperplasia was 5.3% (1/19). ConclusionThe ATEC system can be applied to minimally invasive biopsy of common suspicious calcifications in Chinese women, which is worthy of popularizing.
