目的探讨鼻内镜中鼻甲内移术对嗅觉功能的影响。方法回顾性比较2018年1~10月我科60例鼻内镜中鼻甲内移术(内移术组,n=30)或中鼻甲削薄(削薄术组,n=30)术后6个月嗅觉Sniffin’ Sticks评分。内移术组内镜鼻窦手术完成后以胸腔引流管置于中鼻道前端固定72 h以完成中鼻甲内移术。削薄术组内镜鼻窦手术后切除中鼻甲外侧黏膜及骨质完成中鼻甲削薄术。结果2组术后Sniffin’ Sticks、LundMackay、症状VAS评分与术前比较均明显改善(P<0.05)。削薄术组术后Sniffin’ Sticks评分(27.0±8.6)分,与内移术组术后(29.1±109)分比较差异无显著性(t=1.177,P=0.242)。结论慢性鼻-鼻窦炎行鼻内镜中鼻甲内移术或中鼻用削薄术均可明显改善患者术后嗅觉功能,但2种术式的嗅觉改善效果无差异。
Abstract:
ObjectiveTo investigate the effect of middle turbinate transfer on olfactory function during endoscopic sinus surgery.MethodsA retrospective analysis was made on 60 cases of chronic rhinosinusitis who underwent endoscopic middle turbinate internal transfer(internal transfer group,n=30) or middle turbinate thinning(thinning group,n=30) in our department from January to October 2018. Before and after 6 months of surgery, Sniffin’ Sticks score of olfactory function was tested. In the internal transfer group, the chest tube was placed in the front of the middle nasal passage for 72 hours to complete the middle turbinate transfer at the end of the surgery. In the thinning group, excision of the medial mucosa of the middle turbinate and the bone was done to complete the middle turbinate thinning. ResultsThe scores of Sniffin’ Sticks, LundMackay, and symptom VAS were significantly improved in both groups (P<0.05). There was no significant difference in postoperative scores of Sniffin’ Sticks between the internal transfer group (29.1±10.9) points and the thinning group (27.0±8.6) points(t=1.177, P=0.242).ConclusionsBoth endoscopic inferior turbinate transfer and middle turbinate thinning improve the postoperative olfactory function of chronic rhinosinusitis. There is no difference between the two methods.
