摘要:
目的探讨腹腔镜下胰腺假性囊肿外引流术治疗胰腺假性囊肿的价值。方法2012年7月~2015年7月我院手术治疗25例胰腺假性囊肿，21例胰腺假性囊肿未与主胰管相通者，行腹腔镜下胰腺假性囊肿外引流术，4例囊肿与主胰管相通且>5 cm者，行囊肿胃引流术或囊肿空肠Roux-Y吻合术。结果腹腔镜下胰腺假性囊肿外引流术21例，平均手术时间65.8 min（55~88 min），平均出血量50 ml（20~78 ml），无中转开腹，平均住院时间8.5 d（7~13 d）。囊肿胃引流术2例，手术时间分别为72、75 min，术中出血量分别为70、82 ml，住院时间分别为7、7.5 d。囊肿空肠Roux-Y吻合术2例，手术时间分别为112、122 min，术中出血量分别为120、105 ml，住院时间分别为9、9.5 d。23例随访6~24个月，平均15个月，无囊肿复发。结论胰腺假性囊肿未与主胰管相通采用腹腔镜下胰腺假性囊肿外引流术可取得较好的效果。
Abstract:
ObjectiveTo evaluate the value of laparoscopic extracorporeal drainage of pancreatic pseudocyst in the treatment of pancreatic pseudocyst.MethodsClinical data of 25 patients with pancreatic pseudocysts from July 2012 to July 2015 were retrospectively analyzed. For 21 patients with pancreatic pseudocyst not connecting with the main pancreatic duct, laparoscopic pancreatic pseudocyst external drainage was performed. For 4 patients with cysts connecting with the main pancreatic duct for more than 5 cm, cyst gastric drainage or cyst jejunum Roux-Y anastomosis was performed.ResultsIn 21 patients receiving laparoscopic pancreatic pseudocyst external drainage, the average operative time was 65.8 min (range, 55-88 min), the average bleeding was 50 ml (range, 20-78 ml), and the average hospitalization time was 8.5 days (7-13 days). No conversion to open surgery was required. In 2 cases receiving cyst gastric drainage, the operation time was 72 and 75 min, the intraoperative blood loss was 70 and 82 ml, and the hospitalization time was 7 and 7.5 d. In 2 cases receiving cyst jejunum Roux-Y anastomosis, the operation time was 112 and 122 min, intraoperative blood loss was 120 and 105 ml, and hospital stay was 9 and 9.5 d.The 23 patients were followed up for 6-24 months (mean,15 months), without cysts recurrence.ConclusionFor patients with pancreatic pseudocysts not communicating with the main pancreatic duct, laparoscopic pancreatic pseudocyst external drainage has good results.
