摘要:
目的探讨介入治疗真性肾动脉瘤（renal artery aneurysm,RAA）的疗效。方法回顾分析2012年1月~2015年8月5例真性肾动脉瘤（renal artery aneurysm, RAA）介入治疗的临床资料。男1例，女4例，年龄34~68岁。1例腰酸伴肉眼血尿，1例顽固性高血压伴腰痛，3例无明显症状。均为单发囊性真性肾动脉瘤。行覆膜支架修复术3例，动脉瘤栓塞术2例。结果均一次手术成功。无严重并发症或死亡。1例合并高血压患者置入覆膜支架后恶心、腹痛、发热，3天后缓解，术后1个月在原有降压药治疗基础上血压趋于稳定。血尿患者术后无明显缓解。5例术后3个月复查肾功能，均未出现异常。术后3个月CT随访，未见内漏或动脉瘤残存。6个月复查肾动脉彩超，5例载瘤动脉均通畅。结论介入手术治疗真性肾动脉瘤安全、有效。应综合考虑多因素，选择适合的介入治疗方式。
Abstract:
ObjectiveTo investigate the effect of interventional treatment for true renal artery aneurysm.MethodsFive patients (1 male and 4 females; age: 34-68 years old) with true aneurysm of renal artery received interventional therapy between January 2012 and August 2015 in our hospital. For these patients, 1 case showed backache with gross hematuria, 1 case showed refractory hypertension with backache, and the other 3 cases did not demonstrated obvious symptoms. All of them were single saccular true renal artery aneurysm. Three of them received stent graft repair, and the other 2 cases underwent embolization of aneurysm. ResultsAll the 5 cases got technical success with no serious complications or death. One patient with hypertension showed nausea, abdominal pain, and fever postoperatively, which disappeared 3 days later. The patient maintained the original anti-hypertensive drugs, and the blood pressure turned to be stable at one month postoperation. Patients with hematuria did not demonstrated significant remission after interventional therapy. All the cases were followed up at three months postoperation. Their renal functions were normal and the CT scanning did not find any endoleaks or residual aneurysms. The primary patency rate of renal target artery was 100% (5/5) with renal artery ultrasound examination at the sixth postoperative month.ConclusionsInterventional surgical treatment for renal artery aneurysm is safe and effective. Comprehensive consideration of multiple factors should be carried out, and appropriate intervention techniques should be selected.
