摘要:
目的探讨微创弹性髓内钉(elastic stable intramedullary nailing,ESIN)技术治疗儿童股骨干骨折的疗效。方法回顾性分析2013年1月~2014年9月采用ESIN治疗的30例儿童股骨干骨折的病例资料,闭合复位24例,骨折端小切口切开复位6例。术后1、2、3、6、9、12个月进行随访,X线片检查判断骨折愈合情况。结果30例随访6~24个月,平均12.1月。根据Flynn评价标准,优23例,良6例,可1例,优良率96.7%(29/30)。术后2个月28例可见明显骨痂形成。30例均获得骨性愈合,愈合时间6~14周,平均9周。术后1例有皮肤刺激症状,其余均无髋、膝关节功能受限,无骨不连及肢体短缩畸形。结论ESIN治疗儿童股骨干骨折符合生物学固定原则,骨折愈合良好,创伤小,并发症少,是治疗儿童股骨干骨折的较好方法。
Abstract:
ObjectiveTo investigate the efficacy of minimally invasive elastic stable intramedullary nailing (ESIN) in the treatment of femoral shaft fractures in children.MethodsA total of 30 children with displaced femoral shaft fractures were treated by ESIN from January 2013 to September 2014. There were 24 cases of closed reduction and 6 cases of open reduction through a small incision near the fracture. Follow-ups at 1, 2, 3, 6, 9, and 12 postoperative months were carried out with X-ray examinations to evaluate the bone union.ResultsAl1 the patients were followed up for an average of 12.1 months (range, 6-24 months). According to the Flynn evaluation standards, there were 23 cases of excellent outcomes, 6 cases of good and 1 case of fair, showing an excellent-or-good rate of 96.7% (29/30). The average time of bone union was 9 weeks (range, 6-14 weeks). One patient had irritation at nail insertion site. All the patients had full range of motion in the hip and knee joints and got excellent results without delayed union or non-union, or different length of lower limbs.ConclusionsESIN conforms to principles of biological fixation. With good clinical outcomes, minimal invasion, and fewer complications, it is a better way for the treatment of femoral shaft fractures in children.
