摘要:
目的探讨腹腔镜下脐尿管切除术的效果。方法我院2005年3月~2014年10月行腹腔镜下脐尿管切除术治疗6例脐尿管瘘。上腹部3个穿刺点入路，采用超声刀游离脐尿管，将全部脐尿管连同部分顶部膀胱壁完整切除，20可吸收线连续缝合关闭膀胱裂口，再浆肌层间断缝合包埋。结果手术均获成功，无中转开放手术。手术时间20~50 min，平均30 min。术中出血量10~20 ml，平均15 ml。6例随访3~78个月，平均25.3月，未发现感染、复发、癌变，切口愈合良好，排尿正常。结论腹腔镜下脐尿管切除术治疗脐尿管瘘在严格掌握适应证的前提下是一种有效的方法。
Abstract:
ObjectiveTo discuss the efficacy of laparoscopic resection of urachus for treating urachal fistula. MethodsA total of 6 patients with urachal fistula received threeport laparoscopic resection of the urachus in this hospital from March 2005 to October 2014. After clipping the entire urachus and part of top wall of urinary bladder, the urachus was cut by using an ultrasonic scalpel. The bladder rupture was continuously sutured by 20 absorbable threads, and then the seromuscular layer was sutured and embedded. ResultsAll the operations were completed successfully under a laparoscope without conversions to open surgery. The operation time ranged from 20 to 50 minutes (mean, 30 min). The intraoperative blood loss was 10 to 20 ml (mean, 15 ml). All the 6 patients received a mean of 25.3 months of followup (range, 3-78 months). During the followup period, none of them developed infections, recurrence, or carcinomatous changes. All the patients had normal urination.ConclusionLaparoscopic resection of urachus for treating urachal fistula is an effective method on the basis of strict following surgical indications.
