摘要:
目的探讨腹腔镜辅助胃癌根治术的临床疗效。方法回顾性分析2009年1月~2012年12月由同一组医师完成的82例腹腔镜辅助胃癌根治手术的临床资料，其中cTNM分期Ⅰ期20例，Ⅱ期36例，Ⅲ期26例。结果4例(49%)中转开腹，其中术中出血难以控制和脏器损伤各1例，严重高碳酸血症1例，腹腔肿瘤种植转移1例；其余78例顺利完成腹腔镜辅助胃癌根治术，其中D1+α清扫4例，D1+β清扫13例，D2清扫61例。根治性远端胃大部切除术51例，近端胃大部切除术3例，全胃切除术24例。无手术死亡。手术时间（243．8±31.9）min，术中出血量（121.1±29.1）ml，辅助切口长度(6.3±1.3)cm，清扫淋巴结(26.0±11.8)枚，术后肛门排气时间(3.6±1.6)d，术后恢复流质时间(3.7±1.5)d，术后住院日(12.7±6.0)d。术中和术后并发症各11例（11/78，14.1%）。术后72例(72/78，92.3%)获得随访，中位时间20.7月（3~60个月），未发现戳孔种植，7例术后复发或远处转移，其中4例死亡，3例带瘤生存。结论腹腔镜辅助胃癌根治术安全、可行，围手术期及近期疗效有优势。
