摘要:
目的探讨手助腹腔镜下切除位置深在或巨大肝血管瘤的安全性及可行性。方法2012年5~12月，实施3例手助腹腔镜肝血管瘤切除术。其中2例S6段肝血管瘤位置深在，1例为S7段10.7 cm×6.7 cm×7.0 cm巨大肝血管瘤。术者直接将左手伸入腹腔，辅助完成腹腔镜肝血管瘤切除术。结果3例均顺利完成手术。手术时间分别为150、230和220 min。出血量分别为200、350和500 ml。无中转开腹手术。术后住院时间分别为6、5和8 d。无术后出血及感染等并发症。结论手助腹腔镜下切除位置深在或巨大肝血管瘤是安全可行的。
Abstract:
ObjectiveTo estimate the feasibility and safety of handassisted laparoscopic hepatectomy for huge or deep hepatic hemangioma.MethodsTwo cases of liver hemangioma of S6 segment underwent handassisted laparoscopic hepatectomy from May 2012 to December 2012. Liver hemangioma of S6 segment was deep in site in 2 cases, and there was 1 case of liver hemangioma of S7 segment of about 10.7 cm×6.7 cm×7.0 cm in size. Left hand of the surgeon entered the abdominal cavity directly, assisting in completing laparoscopic resection of liver hemangioma. ResultsAll the surgeries were completed successfully, with the operative time of 150, 230 and 220 min respectively. The blood loss was 200, 350 and 500 ml respectively and postoperative hospital stay was 6, 5 and 8 d respectively. No postoperative complications such as postoperative blood loss or infection occurred.ConclusionHandassisted laparoscopic hepatectomy for huge or deep hepatic hemangioma is safe and feasible.
