摘要:
目的比较宫腔镜下子宫内膜息肉切除术（transcervical resection of polyps，TCRP）后口服安宫黄体酮及宫腔放置曼月乐预防子宫内膜息肉（endometrial polyps, EP）复发的疗效。方法2009年1月~2011年1月92例TCRP术后根据治疗方法不同分为3组，对照组30例，安宫黄体酮组32例，曼月乐组30例，比较3组EP复发及子宫异常出血的改善情况。结果曼月乐组术后EP复发率为0，显著低于对照组16.7%(5/30)和安宫黄体酮组12.5%(4/32)（Z=-2.316，P=0021；Z=-1.986，P=0.047），安宫黄体酮组与对照组EP复发率无统计学差异（Z=-0.462，P=0.644）。3组子宫异常出血改善情况无统计学差异（χ2=0.362，P=0.834）。结论TCRP术后宫腔放置曼月乐可有效预防EP复发，口服安宫黄体酮不能降低EP复发率。
Abstract:
ObjectiveTo compare the curative effects of oral administration of medroxyprogesterone acetate and the placement of LNIUS into uterine cavity on the prevention of recurrence of endometrial polyps (EP) after transcervical resection of polyps (TCRP).MethodsFrom January 2009 to January 2011, 92 patients with EP were divided into 3 groups according to different treatments after TCRP: 30 cases in control group, 32 cases in medroxyprogesterone group and 30 cases in LNIUS group. The recurrence of EP and the improvement of abnormal hemorrhage were compared among the three groups. ResultsThe recurrence rate of the LNIUS group was 0, significantly lower than that of the control group 16.7% (5/30) and the medroxyprogesterone group 125%(4/32) （Z=-2.316，P=0.021；Z=-1.986，P=0.047）. No significant difference could be found in the recurrence rate of EP between control group and medroxyprogesterone group（Z=-0.462，P=0.644）. No significant difference among the three groups was observed in the improvement of abnormal hemorrhage（χ2=0.362，P=0.834）.ConclusionThe placement of LNIUS into uterine cavity after TCRP can effectively prevent the recurrence of EP, while oral administration of medroxyprogesterone acetate cannot reduce the recurrence rate of EP.
