摘要:
目的探讨经脐单孔腹腔镜技术在输卵管妊娠输卵管切除术中的应用价值。方法2010年10月～2011年3月48例因输卵管妊娠行腹腔镜输卵管切除术，其中经脐单孔腹腔镜输卵管切除术22例（单孔组），应用单孔多通道系统，脐部切口长约2.5 cm，常规腹腔镜输卵管切除术26例（三孔组），比较2组患者的手术时间、术中出血量、术后疼痛评分及住院时间等指标。结果2组手术均顺利完成，单孔组手术时间（42.5±26.5）min，显著长于三孔组（28.3±19.7）min(t=2126,P =0.039)，其余观察指标比较无统计学意义（P＞0.05）。术后1个月内2组血βhCG均降至正常范围，无持续性异位妊娠发生。结论经脐单孔腹腔镜输卵管切除术安全、可行，手术切口更易被年轻患者接受，但手术时间长于常规腹腔镜手术。
Abstract:
ObjectiveTo investigate the value of transumbilical singletrocar laparoscopic salpingectomy for the treatment of tubal pregnancy. MethodsSince October 2010 to March 2011, 48 patients with ectopic pregnancy were divided into singletrocar group (n=22) or threetrocar group (n=26) for laparoscopic salpingectomy. In the singletrocar group, an 2.5cm umbilical incision was made. The operation time, intraoperative blood loss, postoperative pain score and hospital stay were compared between the two groups. ResultsThe procedure was completed uneventfully in both the groups. The singletrocar group showed significantly longer operation time than the threetrocar group ［(42.5±26.5) min vs. (28.3±19.7) min, t=2.126, P=0039］. No significant difference was detected in the other indexes between the two groups (P＞0.05). One month after the operation, both the groups showed a normal level of βhCG, and no persistent ectopic pregnancy occurred.ConclusionsTransumbilical singletrocar laparoscopic salpingectomy is safe and feasible, with a more cosmetic incision, which is better accepted by young patients. However the operation time of this technique is longer than that of regular threetrocar operation.
