摘要:
目的探讨经脐单孔腹腔镜阑尾切除术的临床价值。方法我院2009年8月~2010年5月对82例阑尾炎，在脐孔切口1.5 cm应用10 mm 30°腹腔镜，另外置入1~2枚5 mm trocar完成腹腔镜下阑尾切除手术。结果82例手术均获成功，无中转开放手术及改变手术方式。32例经脐单孔双trocar法手术时间25~50 min，平均34 min。住院时间2~5 d，平均3.3 d。50例经脐单孔三trocar法手术时间20~45 min，平均27 min。住院时间2~5 d，平均2.8 d。80例术后随访5~14个月，无切口感染、再发右下腹痛等发生。结论经脐单孔腹腔镜阑尾切除术简单易行、安全、并发症少、恢复快、美容效果较好，但需要特殊的可弯曲腹腔镜操作器械，手术费用较高。
Abstract:
ObjectiveTo evaluate the value of singleport transumbilical laparoscopic appendectomy. MethodsThe clinical data of 82 cases of appendicitis, who received laparoscopic appendectomy in our hospital from August 2009 to May 2010, were analyzed retrospectively. During the operation, we made a 1.5cm umbilical incision so that to place a 10mm 30° laparoscope and another one or two 5mm trocars.ResultsThe operation was completed successfully in all the cases without conversion to open surgery or other approaches. In the patients who received the operation by two trocars (n=32), the operation time ranged from 25 to 50 minutes with a mean of 34 minutes, and their mean hospital stay was 3.3 days (2-5 days). While in the patients who underwent 3trocar laparoscopy, the operation time ranged from 20 to 45 mintues with a mean of 27 minutes, and their mean hospital stay was 2.8 days (2-5 days). Followup was achieved in 80 cases for 5 to 14 months, during which no incisional infection or recurrent right lower abdominal pain occurred. ConclusionSingleport transumbilical laparoscopic appendectomy is convenient and safe with few complications, quick recovery and good cosmetic results, but high cost because flexible laparoscopic instruments are requested for the procedure.
