摘要:
目的探讨腹腔镜下腹主动脉手术的可行性。方法15头家猪,全麻下建立CO2气腹,腹壁放置7个穿刺套管,腹腔镜下向右侧分离降结肠及其系膜,游离并阻断腹主动脉,行人工血管-腹主动脉端侧吻合,以吻合口通畅无出血为手术成功标志。结果6头猪完全手术成功,5头猪主动脉游离成功(未行血管吻合),4头猪死于术中出血。手术成功的猪的主动脉搏动正常,后肢动脉搏动正常。结论腹腔镜下腹主动脉手术可行,腹腔镜技术基本功的训练和术中仔细操作是成功的关键。
Abstract:
ObjectiveTo explore the feasibility of laparoscopic abdominal aorta reconstruction.MethodsFifteen pigs were enrolled in this study. Under general anesthesia, CO2 pneumoperitoneum was established, and then by using 7 trocars, the descending colon and its mesocolon, and the abdominal aorta were freed successively. Afterwards, anastomosis of an artificial vascular graft to the abdominal aorta was completed, which was evaluated by its patency and degree of bleeding.ResultsLaparoscopic aortic reconstruction was succeeded in six pigs, and partially completed in 5 pigs (no vascular anastomosis). Four pigs died of intraoperative hemorrhage. The pulse of the aorta and arteries supplying the posterior limb were normal in the 6 successful cases.ConclusionsLaparoscopic aortic reconstruction is feasible in animals. The basic laparoscopic skills and elaborate manipulations are crucial to the success of the operation.
