摘要:
目的 探讨局麻下B超引导经皮肾造瘘输尿管镜钬激光治疗复杂性肾结石的可行性.方法 对32例复杂性肾结石,局麻下B超引导穿刺,扩张至F16时留置鞘,F8/9.8输尿管镜作为操作镜,采用钬激光碎石,分1～3次碎石处理,间隔3～5 d.结果 穿刺均成功,末次碎石后结石排净率4周75%(24/32),12周94%(30/32),无严重出血、感染等并发症,无中转开放手术.结论 局麻下B超引导经皮肾造瘘输尿管镜钬激光治疗复杂性肾结石安全高效,对设备技术要求不高,切实可行.
Abstract:
Objective　To explore the feasibility of B-ultrasound guided ureteroscopic holmium laser nephrolithotomy via percutaneous nephrostomy for the treatment of complex renal calcul.i　M ethods　A total of 32 cases of complex renal calculiwas included in the study. Under local anaesthesia and B-ultrasound guidance, a percutaneous nephrostomy was performed untill a F16 catheter could be introduced, with the sheath indwelling in the tract. Then under a F8/9. 8ureteroscopy, holmium lasernephrolithotomy was conducted for1~3 fractionswith an interval of3~5 d.　Results　The renal puncture was successfully accomplished in all the cases. The stone-free ratewas 75% (24/32) at4 weeks, and 94% (30/32) at12 weeks after procedure. No serious hemorrhage or infection happened. No conversion to open surgerywas required.　Conclusions　Under local anaesthesia and B-ultrasound guidance, ureteroscopic holmium lasernephrolithotomy via percutaneous nephrostomy for the treatmentof complex renal calculi is safe, effective, and feasible.
