摘要:
目的探讨脊柱后路内镜下椎间盘摘除术(microendoscopic disectomy,MED)治疗青少年腰椎间盘突出症的疗效. 方法回顾性分析我院2000年2月～2004年8月MED治疗25例青少年腰椎间盘突出症的临床资料,汉化Oswestry功能障碍指数(Oswestry disability index,ODI)评价术前及术后随访时的症状、功能改善情况,改良Macnab标准评价临床疗效.结果 1例因出血中转开放手术.手术时间35～65 min,(44.8±9.0) min;术中出血量30～80 ml,(51.3±14.6 )ml;术后住院时间6～10 d,(7.5±1.0) d.切口均一期愈合,无硬膜撕裂、神经根损伤、椎间隙感染、腹腔血管损伤并发症.22例随访时间7～57个月,(33.4±17.8)月.术前ODI指数(46.2±8.5)%,术后随访时ODI指数(1.8±3.0)%,ODI指数改善率(44.4±9.2)%,术前、术后ODI指数经配对t检验差异有显著性(t=21.61,P=0.00).改良Macnab标准评价临床疗效,优19例,良3例,优良率100%. 结论 MED治疗青少年腰椎间盘突出症创伤小、恢复快、疗效好,是治疗青少年腰椎间盘突出症安全、有效的方法.
Abstract:
Objective　To explore clinical results of posterior spinal approach microendoscopic disectomy (MED) for the treatment of lumbar disc herniation in adolescents.　M ethods　A totalof25 consecutive patients treated byMED from February 2000 to August2004 in this hospitalwere analyzed retrospectively. Clinical results were assessed with the modifiedMacnab criterion. Pre-and post-operative symptoms and functional stateswere evaluated by the Chinese version Oswestry Disability Index (ODI).　Results　A conversion to open procedure was required in 1 patient. The operating time was 35~65 minutes (mean, 44. 8±9. 0 minutes); the estimated blood loss during operationwas 30~80 ml (mean, 51.3±14.6 ml); the postoperative hospital staywas 6~10 days (mean, 7·5±1.0 days). All incisionshealed by first intention. Therewere no dural tears, nerve root injuries, intervertebral space infections, or great vessel injuries. Twentg-two patients were followed for 7 ~57 months (mean, 33. 4±17. 8 months). There were significant differences between preoperativeODI (46. 2%±8. 5% ) and postoperative ODI (1. 8%±3. 0% ). The improvement rate ofODIwas 44·4%±9.2% (t=21·61,P=0.00). Clinical results assessmentby themodifiedMacnab criterion revealed“excellent”in 19 patients and“good”in 3 patients, the rate of excellent or good results being 100%.　Conclusions　Microendoscopic disectomy can be performed safely and effectively for lumbar disc herniation in adolescents, resulting in little trauma, fast recovery, and excellent clinical results.
