摘要:
目的探讨子宫下段妊娠的临床特点及治疗方法. 方法回顾性分析2005年1月～2006年2月6例子宫下段妊娠的临床资料. 结果 5例有剖宫产史,另1例妊娠过频.其中2例甲氨蝶呤(MTX)配合子宫动脉栓塞术(uterine artery embolization,UAE),1例行UAE,1例单纯行MTX治疗,2例行全子宫切除术. 结论早孕合并子宫瘢痕以及妊娠过频的患者,再次受孕有发生子宫下段妊娠的危险性,在行人工流产时易发生难以控制的大出血,应术前明确诊断.MTX配合UAE是一种安全可靠的治疗方法.
Abstract:
Objective　To discuss the diagnosis andmanagement for lower uterine segmentpregnancy.　M ethods　Clinical data of6 cases of loweruterine segmentpregnancy between January2005 and February2006 were retrospectively analyzed.　Results　 Five patients had a history of caesarean section, while another one had undergone repeated pregnancy. Uterine artery embolization (UAE) in combination with methotrexate (MTX) injection was carried out in 2 cases, and uterine artery embolization only, MTX injection only, and totalhysterectomywere employed in 1 case, 1 case, and 2 cases, respectively.　Conclusions　Previous caesarean section and repeated pregnancy are risk factors of the occurrence of lower uterine segment pregnancy. Uterine artery embolization in combination withmethotrexate injection is a safe and reliablemanagement option.
