摘要:
目的探讨腹腔镜辅助阴式全子宫切除术的效果、手术要点. 方法对121例子宫肌瘤、宫颈上皮内瘤样病变Ⅲ级、子宫腺肌瘤、卵巢肿瘤、子宫内膜不典型增生等有子宫切除指征的患者行腹腔镜辅助阴式全子宫切除术. 结果 117例手术成功,由于粘连严重中转开腹4例,成功率96.7%(117/121).平均手术时间95 min(58～192 min),术中平均出血量120 ml(100～250 ml),术后住院5 ～8 d. 结论腹腔镜辅助阴式全子宫切除术创伤小,肠道干扰少,恢复快.手术成功与子宫大小、形状、活动度、阴道的容量及术者的经验和技术有关.
Abstract:
Objective　To investigate effects and techniques of laparoscopic assisted transvaginal hysterectomy (LAVH).　 M ethods　Laparoscopic assisted transvaginal hysterectomywas carried out in 121 patientswith hysteromyoma, cervical intraepithelial neoplasia gradeⅢ, uterine adenomyoma, ovarian tumor, or atypicalhyperplasia of endometrium, whichwere all indications foruterus resection.　Results　LAVH was performed successfully in 117 cases, whereas a conversion to open surgerywas required in 4 cases because of serious adhesion, the success rate being96·7% (117/121). Themean operating timewas95min (range, 58~192min). Themean amount of intraoperative blood losswas 120 ml (range, 100~250 ml). The postoperative hospital staywas 5~8 days.　 Conclusions　LAVH was associated withmild trauma, slight intestinal disturbance, and quick recovery. Whether ornot an operation is successful is related to the size, shape andmobility of the uterus, vaginal space, and operator’s skills and experience.
